REGISTER FOR PATIENT ACCESS

If you don't already have a Patient Access account, please follow the steps below to sign up. Please also visit this link to view instructions and additional information on the Patient Access website.

  1. If you have a compatible smartphone or tablet, first download the app, available through the Apple Store or on Google Play. If you don't have access to a smartphone or tablet, continue to Step 2.

  2. Register for an account on the Patient Access site. 

  3. Once you have set up an account, please send one form of photographic ID to chartfield.admin@nhs.net. 

  4. Once we have received your ID, we will then send you the additional login information. 

ONLINE ACCESS

Prescriptions and test results can be organised through online access.
